As required by the Health Homes Program, our Case Managers conduct monthly visits to ensure services are appropriate, effective, and meeting your child’s evolving medical and developmental needs. These visits allow us to identify concerns early, address gaps in care, and ensure your child is always receiving the best possible support at home.
